How to Apply for a Philippine Passport – DFA - NCR East Megamall – Part 1


PART 1:  SETTING AN APPOINTMENT THROUGH DFA PASSPORT APPOINTMENT SYSTEM

Getting a Philippine passport used to be very complicated but with the help of technology applying for a Philippine passport is now efficient and world class.  You can now make an appointment online through the DFA website which is applicable for first time applicant, renewal and lost passport.

STEP 1:  PREPARE ALL THE DOCUMENTS NEEDED FOR PASSPORT APPLICATION.

General Requirement

1. Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by NSO. Transcribed Birth Certificate from the LCR is required when entries in NSO Birth Certificate are blurred or unreadable. (Report of Birth duly authenticated by NSO if born abroad).


2. Valid picture IDs and supporting documents to prove identity.

NOTE:
For first time applicants, the ORIGINAL BIRTH CERTIFICATE will be LEFT at DFA as file copy or reference. For renewal, they only get the photocopies of IDs and requirements.


STEP 2: MAKE AN ONLINE APPOINTMENT WITH DFA PASSPORT APPOINTMENT SYSTEM.

DFA NCR - East (SM Megamall) is open from 10:00 AM - 7:00 PM (Mondays - Saturdays) and setting an appointment is a service which is free of charge.

Go to DFA website click "Set an Appointment" then select site nearest your place of residence or place of work to set an appointment. You can also see and refer to the location map of the DFA branches.
A Location Map is also available for travel reference.

SET AN APPOINTMENT HERE. 

Click  but for Family Applications, to help ensure that your appointment dates are all the same, please use Family Application Form.

FILL OUT APPLICATION FORM COMPLETELY.


If you want to see the requirements again click the “SHOW ALL REQUIREMENTS AGAIN” button otherwise you can proceed by clicking the “I’M READY TO PROCEED” button.

Then, you can pick a date for your appointment, wait for the possible slot to appear and click the slot you want. (TIP: Choose an earlier time as much as possible because there will be no backlog yet in the processing of applications thus you can avoid longer queues or GET A THURSDAY SLOT. I’m used to process things/papers on Thursday because in my personal experience there are fewer lines during Thursdays whether in payment centers or government offices.)

NOTE: If you place multiple appointments, your previous appointments will be automatically canceled.

Check your e-mail to confirm your appointment.
You’ll be provided with a link to confirm your appointment and then print your application form.

TAKE NOTE: Use LONG BOND PAPER when printing your application form also DO NOT SIGN the application form. You need to sign it with the presence of a DFA officer in the processing window.

Be sure to bring your application form with you on your appointment date. Applicants will not be admitted at the entrance gate without valid appointment and application forms.

Please have all your original documents and IDs PHOTOCOPIED before going to DFA on your appointment date. Applicants will not be processed if they do not have complete set of photocopies of their documents.
